Join a dedicated team providing comprehensive, patient-centered care in a Medical Home Model as an Advanced Practice Provider (PA or NP) in Prineville, Oregon.
Deliver diagnosis, treatment, and management for patients of all ages, collaborating with behavioral health, pharmacy, care managers, and specialists.
Full-time position with a 40-hour workweek, including a minimum of 36 patient contact hours and participation in clinic call rotation (evenings, weekends, holidays).
Work closely with physicians and community health educators, practicing evidence-based family medicine focused on prevention.
Responsibilities include EHR documentation, coding, billing, and maintaining ongoing education and certifications.
Requirements: Master’s in Physician Assistant Studies or Nursing (Doctorate preferred), unrestricted Oregon PA/NP license, national certification, DEA & BLS (ACLS for sedation), 2+ years outpatient family medicine experience, and familiarity with EHR systems (Epic preferred).
