Job Description
The position is for a compassionate Family Nurse Practitioner (FNP) providing primary and specialty care across outpatient, inpatient, and community settings.
Conduct thorough patient assessments, develop personalized treatment plans, and manage acute and chronic conditions, including geriatrics and complex cases. Utilize EHR systems like Epic or Athenahealth for documentation, coding, and billing. Coordinate care through discharge planning, home care, hospice, and post-acute services. Engage in telehealth consultations ensuring HIPAA compliance. Educate patients on health topics tailored to individual needs. Require at least 3 years of diverse clinical experience, familiarity with EMR/EHR systems, and knowledge of CPT and ICD coding. Experience with vulnerable populations and behavioral health is a plus. This part-time role offers $55,000-$80,000 annually, 20-25 hours weekly, with paid time off, in a dynamic healthcare environment focused on patient-centered care.
