Nurse Practitioner- Johnstown, NY

Job Description

We are seeking a compassionate Nurse Practitioner (NP) to join our Skilled Nursing Facility in Johnstown, NY in a full-time role with part-time or per diem options. The NP will collaborate with healthcare teams to provide high-quality care, including assessments, treatment planning, managing medical conditions, and coordinating interdisciplinary care.

Conduct patient assessments and develop treatment plans

Manage acute and chronic conditions in partnership with physicians

Order, interpret tests, prescribe medications, and document in EHR

Participate in team meetings, family conferences, and quality improvement activities

Ensure compliance with regulations and support nursing staff

Qualifications: NY NP license, AGNP/FNP certification, DEA registration, BLS (ACLS preferred), 1-2 years geriatrics or long-term care experience ideal.
