Job Description

We are seeking a compassionate Nurse Practitioner (NP) for a Skilled Nursing Facility in Johnstown, NY. This full-time position offers flexible scheduling, including part-time or per diem options, with a typical Monday–Friday schedule and some weekend/holiday coverage.

Conduct comprehensive assessments and develop individualized treatment plans.

Manage acute and chronic conditions in collaboration with physicians.

Order, interpret tests, prescribe medications, and monitor patient progress.

Coordinate care with interdisciplinary teams and provide guidance to staff.

Ensure documentation compliance and participate in quality improvement activities.

Qualifications include NY licensure, AGNP/FNP certification, DEA registration, and 1-2 years of geriatrics experience preferred.
