Job Description

We are seeking a compassionate Nurse Practitioner (NP) to join a Skilled Nursing Facility in Johnstown, NY. This full-time role offers flexible scheduling, including part-time or per diem options, with a typical Monday–Friday schedule and some weekend/holiday coverage.

The NP will collaborate with healthcare teams to perform assessments, develop treatment plans, manage medical conditions, order tests, prescribe medications, and ensure compliance with regulations. They will participate in team meetings, support nursing staff, and contribute to quality improvement activities.

Qualifications include an active NY NP license, certification as an Adult-Gerontology or Family Nurse Practitioner, DEA registration, and experience in geriatrics or long-term care preferred. Strong clinical, communication, and organizational skills are essential.
