Job Description
Hospice of Humboldt is seeking a skilled and compassionate Nurse Practitioner or Physician Assistant to join our Palliative Care team.
The Palliative Care NP/PA works independently and in collaboration with the interdisciplinary team, the NP/PA delivers comprehensive, patient-centered care focused on symptom management and quality of life. Responsibilities include conducting assessments, developing and updating care plans, completing regulatory visits (intake, follow-up, recertification), and facilitating transitions to hospice when appropriate.
*Required Education/Credentials for Nurse Practitioners (NP):*
* Master’s degree in nursing from an accredited college or university
* Active NP and RN licensure in California
*Required Education/Credentials for Physician Assistants (PA):*
* Bachelor’s degree or above from an accredited college or university.
* Graduate of a PA program accredited by the Accreditation Review Commission on Education for the Physician Assistant, Inc.
* Active PA licensure in California
*Required Experience:*
* Minimum two years’ experience as an advanced practice provider.

* Strong clinical assessment skills.
* Commitment to interdisciplinary collaboration.
* Compassionate and patient-centered approach to care.
* Excellent communication and counseling skills with patients, families, and interdisciplinary teams.
* Ability to work independently while fostering collaboration.
* Proficiency with electronic medical records and basic computer systems.
* Organizational and time-management skills to prioritize tasks and meet deadlines.
* Valid driver’s license and reliable transportation.
*Preferred Qualifications*
* Prior palliative care experience.
* Board certification:
* PAs: Certification as a PA by the National Commission on Certification of Physician Assistants (NCCPA)
* NPs: National Board Certification as a NP from an agency accredited by the American Board of Nursing Specialties (ABNS).

Pay: $59.52 – $72.75 per hour
Expected hours: 20.0 – 29.0 per week
