Job Description

Seeking an Advanced Practice Provider (PA or NP) passionate about compassionate, high-touch wound care in a mobile, home-based setting.

This full-time role offers flexible Monday-Friday hours, no on-call, weekends, evenings, or holidays, with daily travel to patients’ homes (mileage reimbursed).

Responsibilities include assessing patients, developing individualized care plans, performing advanced treatments, managing medications, and collaborating with care teams to ensure quality wound care.

Qualifications:

  • Graduate of an accredited NP or PA program
  • At least 1 year of experience as an AP provider
  • Wound care experience is a plus
  • Board certification and valid license in Minnesota
  • Ability to travel 75%
