Job Description

Join a dedicated healthcare team providing exceptional post-acute care services within the community. This Advanced Practice Provider role involves managing patient care, collaborating with staff, conducting rounds, writing orders, and maintaining accurate records. Comprehensive training, shadowing, and supervision ensure support and preparedness.


  • Coordinate and deliver quality patient care, communicate effectively with team members, and facilitate diagnostics and treatments.
  • Maintain timely documentation in electronic medical records and ensure compliance with healthcare policies.
  • Utilize strong communication, empathy, organizational, and problem-solving skills to meet patient needs.

    • Requirements include a Nurse Practitioner license, DEA certification, and preferred experience in geriatrics, internal medicine, ICU, emergency medicine, or hospitalist settings.
