Job Description

Join a dedicated healthcare team providing exceptional post-acute patient care within the community. This Advanced Practice Provider role involves managing and coordinating patient care, conducting rounds, documenting in electronic medical records, and collaborating with staff and physicians.


  • Coordinate and deliver high-quality patient care while maintaining accurate documentation and communication.
  • Participate in daily patient rounds, order tests and procedures, and contribute to care planning.
  • Ensure compliance with policies and maintain detailed records for billing and reporting.

    • Ideal candidates possess excellent communication, empathy, organizational skills, and a team-oriented attitude. Preferred experience includes geriatrics, internal medicine, ICU, emergency medicine, hospitalist, or general practice. Must hold a Nurse Practitioner license and DEA certification.
