Job Description

We are seeking a qualified healthcare professional to provide comprehensive geriatric post-acute care across various settings such as hospitals, rehab centers, nursing facilities, assisted living, and patients’ homes.

Responsibilities include

  • Providing consulting and rounding services.
  • Maintaining active privileges at assigned facilities.
  • Following Medicare, state, and employer policies for service delivery and billing.
  • Completing timely documentation for billing and patient records.
  • Supervising Advanced Practice Providers as assigned.

    • Requirements include a Texas Medical Board License, DEA, ACLS, and professional credentials. Preferred qualifications are a Master’s degree and certifications such as Family Nurse Practitioner or AGCNS-BC.
