Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is a full-time APRN or Physician Assistant working independently in PALTC facilities, collaborating with staff and leadership to enhance patient care.


  • Manage comprehensive medical needs, including assessments, diagnoses, treatments, and preventive services.
  • Make independent clinical decisions within scope, escalating as needed, and serve as the primary provider.
  • Build strong interdisciplinary relationships to coordinate holistic patient care and communicate with families and external providers.
  • Ensure compliance with regulations, maintain accurate documentation, and participate in quality initiatives.
  • Participate in infection control, health education, and safety protocols.
  • Work effectively with minimal supervision, demonstrating strong clinical and communication skills, proficiency in EHR and telemedicine, and physical ability to respond quickly in emergencies.
  • Qualifications include a relevant degree, 2+ years experience, board certification, and licensure as an APRN or PA.
