Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is a full-time APRN or Physician Assistant working independently in PALTC facilities, collaborating with staff and leadership to enhance patient care.
Manage comprehensive medical needs, including assessments, diagnoses, treatments, and preventive services. Make independent clinical decisions within scope, escalating as needed, and serve as the primary provider. Build strong interdisciplinary relationships to coordinate holistic patient care and communicate with families and external providers. Ensure compliance with regulations, maintain accurate documentation, and participate in quality initiatives. Participate in infection control, health education, and safety protocols. Work effectively with minimal supervision, demonstrating strong clinical and communication skills, proficiency in EHR and telemedicine, and physical ability to respond quickly in emergencies. Qualifications include a relevant degree, 2+ years experience, board certification, and licensure as an APRN or PA.
