Nurse Practitioner – Spanish Required

Job Description

We are seeking a Nurse Practitioner to deliver equitable, value-based primary care to Medicare-aged adults in outpatient settings.

Provide comprehensive primary care, conduct office visits, wellness exams, and health assessments.

Coordinate care with specialists, testing facilities, and care teams.

Lead population health initiatives and assist with outreach and phone triage.

Educate patients on health conditions and care plans, and conduct home visits as needed.

Collaborate closely with a multidisciplinary team including physicians, nurses, and support staff.

Qualifications: Master’s degree or higher in nursing, active NP license, DEA license, and certification in relevant specialty. Preferred experience in primary care and understanding of population health documentation.
