About the Role:
- Perform assessments and develop personalized wound care plans
- Provide bedside wound care in skilled nursing and assisted living facilities
- Lead wound assessment, treatment, and advanced procedures using innovative technology
- Collaborate with facility teams and families for holistic care decisions
- Maintain accurate charting in EMR and support quality initiatives
Qualifications:
- Active Pennsylvania Nurse Practitioner license or Physician Assistant license (board-eligible not accepted)
- Prescriptive authority and DEA license required within training period
- Minimum 2 years wound care experience, with long-term care preferred
- Willingness to travel within designated counties
- Proficient in EMR and mobile documentation
- Strong interpersonal and collaborative skills
