Job Description
Our Nurse Practitioners serve communities across South Carolina by providing direct clinical care in residential, assisted living, and independent-living facilities.
They offer primary and palliative care for home-bound patients, those with transportation challenges, and the geriatric population.
Responsibilities include collaborating with an interdisciplinary team, conducting physicals, educating patients on preventive care, managing chronic illnesses, ordering and reviewing tests, developing treatment plans, prescribing medications, and coordinating patient-centered care.
The role involves transitional care for hospice services, medication management, documentation in EHR, and attending CE events. It requires a flexible 40-week schedule, local travel, and relocation to Lancaster, SC before starting.
Requirements include a Master’s degree or postmaster certification as an APRN, current SC licensure, ANCC or AANP certification, CPR and DEA licenses, and at least 2 years of experience preferred.
Compensation starts at $100,000 annually.
