About the Position

LocumJobsOnline is working with Lancesoft Inc to find a qualified Internal Medicine MD in West Springfield, Massachusetts, 01089!

• Community-based care model serving frail and elderly population
• Leadership role overseeing clinical operations and quality programs
• Administrative supervision by Executive Director
• Focus on maintaining independence for seniors in community setting

Job Responsibilities
• Provide medical direction and supervision of participant care
• Oversee clinical outcomes and QAPI program
• Develop and maintain medical policies, procedures, and protocols
• Supervise Primary Care Providers (PCPs) and Advanced Practice Providers (APPs)
• Conduct H&Ps, reassessments, and treatment planning
• Evaluate PACE applicants and participate in intake decisions
• Lead interdisciplinary team collaboration and case conferences
• Educate participants and families on chronic disease management
• Perform physical exams and order diagnostic testing
• Participate in provider on-call rotation as required
• Establish leadership plans to achieve operational goals

Requirements

• Active Massachusetts Physician License – REQUIRED
• Board Certified in Internal Medicine or Family Practice – REQUIRED
• Board Certification in Geriatrics – PREFERRED
• Minimum 1 year experience with frail/elderly population – REQUIRED
• Previous supervisory experience – REQUIRED
• DEA Registration – REQUIRED
• CSR/CDS – REQUIRED
• NPDB Self-Query (current month) – REQUIRED
• Strong chronic disease management expertise
