Job Description

Seeking a dedicated MD or DO primary care physician to provide on-site, relationship-driven care in senior living communities across the Greater Twin Cities area.


  • Conduct regular rounds in assisted living and memory care settings.
  • Perform comprehensive assessments and manage complex health conditions.
  • Create personalized care plans collaboratively with patients and families.
  • Coordinate with care teams to ensure seamless service.
  • Educate residents and loved ones on care options and goals.
  • Proactively manage chronic conditions to reduce hospital visits.

    • Qualifications include active Minnesota medical license, preferred board certification in Internal Medicine, Family Medicine, or Geriatrics, with prior senior living or rounding experience. Strong communication skills and a valid driver’s license are required.
