Job Description

We are seeking a full-time Family Nurse Practitioner dedicated to providing community-based primary care in Hampton Roads.

Provide comprehensive, evidence-based care to adult patients, focusing on preventive health, chronic disease management, and acute care.

Collaborate with patients and care team members to develop personalized care plans respecting diverse backgrounds.

Conduct physical exams, diagnostics, and prescribe medications using onsite lab and pharmacy services.

Lead efforts in health promotion, participate in quality improvement activities, and support organizational initiatives.

Achieve clinical and engagement benchmarks to improve patient outcomes.

Requirements: Board-certified NP with primary care experience, valid license, 3-5 years of primary care, EMR and team-based care experience, CPR certification, and documentation of vaccinations and TB testing.

Benefits include health, dental, vision insurance, and paid time off. This in-person role offers $110,000-$140,000 annually.
