Job Description

We are seeking experienced Family Medicine / Internal Medicine Physicians to provide primary and preventive care in skilled nursing and long-term care facilities.


  • Partner with nursing staff and facility leadership to coordinate patient care.
  • Manage patient health alongside mid-level providers, with specialty support available.
  • Attend facility meetings and ensure accurate documentation using EMR.

    • Qualifications include a current, unrestricted TX MD/DO license and board certification in Internal Medicine, Family Medicine, or Geriatrics.
