Job Description
We are seeking experienced Nurse Practitioners and Physician Assistants to provide in-home and virtual health assessments for members of various health plans nationwide.
Assess current health status, identify care gaps, and support care management to improve clinical outcomes.
Enjoy flexible scheduling Monday–Sunday, 8am–8pm, with competitive pay ranging from $100 to $120 per hour, plus completion bonuses and mileage reimbursement.
Conduct comprehensive evaluations, including medication reviews, physical exams, and health discussions, in members’ homes or virtually, fostering personalized care and education.
Qualifications include an active, unencumbered license, board certification through AANP or ANCC, and current CPR certification. Home health experience and multilingual skills are pluses.
