Licensed Clinical Social Worker (LICSW)

Job Description

We are seeking a part-time LICSW Social Worker specializing in Geriatric Psych to support long-term care facilities in Minneapolis, MN.

Provide direct psycho-social services, including evaluations, diagnosis, and treatment for nursing home residents through individual, group, and family therapies.

Collaborate with an interdisciplinary team to ensure quality care and continuity.

Manage 10-15 patient contacts daily with flexibility for workload adjustments.

Maintain knowledge of Medicare guidelines and proper documentation.

Work autonomously with clinical support, utilizing electronic medical records.

Qualifications include an LICSW license, 1+ years of clinical social work experience, and a passion for geriatric care. Preferred experience in healthcare settings and Medicare documentation is a plus.
