Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is a full-time APRN or PA working independently in PALTC facilities to enhance care quality, improve patient outcomes, and reduce hospitalizations and costs.
Manage comprehensive patient care, including assessments, diagnoses, treatments, and preventive services.
Work autonomously, making clinical decisions within scope and escalating as needed.
Collaborate with facility leadership, interdisciplinary teams, and families to ensure holistic care.
Ensure compliance with regulations, maintain accurate documentation, and participate in quality improvement.
Participate in infection control, health promotion, and safety protocols.
Require at least 2 years of experience, relevant degree, and board certification, with long-term care experience preferred.
Proficiency with EHR, telemedicine, and strong communication skills are essential.
