Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is a full-time APRN or PA working independently in PALTC facilities to enhance care quality, improve patient outcomes, and reduce hospitalizations and costs.

  • Manage comprehensive patient care, including assessments, diagnoses, treatments, and preventive services.
  • Work autonomously, making clinical decisions within scope and escalating as needed.
  • Collaborate with facility leadership, interdisciplinary teams, and families to ensure holistic care.
  • Ensure compliance with regulations, maintain accurate documentation, and participate in quality improvement.
  • Participate in infection control, health promotion, and safety protocols.
  • Require at least 2 years of experience, relevant degree, and board certification, with long-term care experience preferred.
  • Proficiency with EHR, telemedicine, and strong communication skills are essential.
