The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an in-person role for an APRN or PA working independently in PALTC facilities.

They must work independently within scope, ensure compliance, maintain accurate records, participate in quality initiatives, and promote health and safety.
Essential skills encompass strong clinical judgment, communication, teamwork, proficiency with EHR and telemedicine, and adaptability for emergency situations.
Qualifications include a relevant degree, 2+ years experience (long-term care preferred), and board certification.
The full-time position offers benefits and a salary range of $66,948–$120,000.
