Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA working independently in PALTC settings to enhance patient care, improve outcomes, and reduce hospitalizations and costs.


  • Provide comprehensive assessments, diagnose, treat, and manage acute and chronic conditions, and deliver preventive services.
  • Work independently within scope, making clinical decisions and escalating as needed, while collaborating with facility leadership and interdisciplinary teams.
  • Ensure compliance with regulations, maintain accurate documentation, and participate in quality initiatives.
  • Participate in infection control, health education, and safety protocols.
  • Demonstrate strong communication, leadership, and technology skills, with the ability to work independently and respond swiftly to emergencies.

  • Qualifications include a relevant degree, minimum 2 years of experience (preferably in long-term care), and board certification. The role is full-time, in-person, with competitive benefits and a salary range of $66,948 to $120,000 annually.
