Nurse Practitioner

Job Description

Our Nurse Practitioners provide primary and palliative care to local communities across South Carolina, including residential homes, assisted living, and independent-living facilities.

They serve home-bound and geriatric patients, offering medical assessments, chronic disease management, education, and counseling.

Responsibilities include collaborating with interdisciplinary teams, developing treatment plans, prescribing medications, reviewing test results, and coordinating patient-centered care.

They manage medication and lab orders, document in EHR, and support transitional hospice care.

Work is scheduled Monday-Friday over 40 weeks with local travel required.

Requirements include a Master’s in Nursing or APRN certification, current SC licensure, certification (ANCC/AANP), CPR, DEA license, and at least 2 years of experience preferred.

Salary starts at $100,000 annually.

Relocation to Conway, SC is required before starting.
