Job Description

Seeking a Full-time or Part-time Nurse Practitioner or Physician Assistant to provide primary care in Skilled Nursing and Assisted Living Facilities in Farmington, ME, and surrounding areas.

  • Perform primary care visits, medication reconciliation, and annual wellness exams 3 to 4 days weekly.
  • Examine patients, review medical records, order diagnostic tests, and diagnose health conditions.
  • Prescribe medications, develop treatment plans, and educate patients and families on health management and prevention.
  • Maintain accurate records, oversee patient health, and promote good health practices.
