Job Description
We are seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide in-person psychiatric assessments, medication management, and medication dose reduction strategies in long-term care facilities within your community.
The role involves conducting comprehensive evaluations, developing treatment plans, prescribing psychotropic medications, and managing chronic mental health conditions such as depression, anxiety, bipolar disorder, schizophrenia, and dementia-related behaviors.
You will collaborate with interdisciplinary teams, ensure accurate documentation, and build strong relationships with facility staff to support residents’ mental health needs.
