Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

Job Description

We are seeking an experienced Nurse Practitioner to provide primary care in Skilled Nursing Facilities, Long-Term Care, and Assisted Living Facilities across Minnesota.

Deliver comprehensive assessments, manage acute and chronic conditions, and develop treatment plans.

Order diagnostics, prescribe medications, and educate patients, families, and staff.

Collaborate with care teams to ensure seamless, patient-centered care.

Qualifications include: MN NP license, board certification, and preferably 1+ year of experience in similar settings.
