Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

Job Description

We are seeking an experienced Nurse Practitioner to provide high-quality, patient-centered primary care in Skilled Nursing Facilities, LTC, and ALFs.

Deliver comprehensive assessments, manage acute and chronic conditions, and develop treatment plans.

Order diagnostics, prescribe medications, and educate residents, families, and staff.

Collaborate with care teams to ensure seamless patient support and documentation.

Qualifications include: Minnesota NP license, board certification, and preferred experience in geriatric or post-acute care.
