This Clinical Nurse role involves providing skilled nursing care in a home health setting, aligned with professional standards and under the supervision of a Nurse Manager/Director.
Participate in patient and caregiver education, assessing learning needs and readiness. Evaluate patient outcomes, collaborate with the interdisciplinary team, and communicate effectively for continuity of care. Deliver competent care to patients with complex conditions, prioritizing safety and psychosocial needs. Assist in developing professional goals, promote collaboration, and support quality improvement initiatives. Conduct thorough patient assessments, anticipate discharge needs, and foster family advocacy. Plan and coordinate care with families and multidisciplinary resources, considering cultural factors. Engage in ongoing research and professional development to enhance practice.
Requirements include a nursing degree, North Carolina RN licensure, BLS certification, with preferred ACLS and PALS. One year of nursing experience caring for acute or chronic patients is required. This full-time, onsite position offers day hours within Nash County.
