Job Description
Part-time/PRN licensed Speech Language Pathologist (SLP) position assessing, diagnosing, and treating communication and swallowing disorders for patients in their homes.
Responsibilities include conducting initial assessments, developing individualized treatment plans, collaborating with the care team to ensure continuous care, and documenting patient progress in the health information system.
Work with patients experiencing cognitive-communication and swallowing issues due to stroke, brain injury, illness, surgery, injury, or dementia. Must have an active SLP license and a valid driver’s license.

Relocation to State College, PA, required before starting work. This role involves travel and on-the-road service delivery.
