Speech Language Pathologist (Home Health) (Part time/PRN)

Job Description

We are seeking a licensed Speech Language Pathologist (SLP) for a part-time/PRN role to assess, diagnose, and treat communication and swallowing disorders in patients’ homes.

Responsibilities include:

• Conducting initial assessments
• Developing individualized treatment plans
• Collaborating with the care team to ensure continuity of care
• Documenting patient progress in the health information system

Requirements:

• Current SLP license
• Valid driver’s license
• Ability to relocate to State College, PA before starting
