Job Description

We are hiring a Licensed Clinical Social Worker (LCSW) specializing in geriatric behavioral health for flexible part-time or full-time roles in Union Grove, WI.

  • Provide direct psycho-social assessments, diagnosis, and treatment for nursing home residents, including individual, group, and family therapy.
  • Collaborate with interdisciplinary teams to ensure quality care and continuity.
  • Manage 10-15 patient visits per day with flexibility for scheduling.
  • Maintain documentation per Medicare guidelines and utilize electronic medical records.

    • Qualifications include a Master’s degree, LCSW license, 1-2 years of experience, and proficient communication and tech skills.
