The Wound Care Nurse Practitioner (NP) reports to the Director of Nursing and supervises nurses and medical assistants.
Job Summary: The NP provides specialized wound care in home, nursing home, and assisted living settings. Responsibilities include assessments, care plan development, patient and caregiver education, and collaboration with healthcare teams to promote healing.
Qualifications: Master’s or Doctoral degree in Nursing, current NP license, wound care certification within 6 months, minimum 2 years of relevant experience, and ability to work independently.
Job Factors: Requires physical effort, field-based travel, and adherence to safety protocols. Must have reliable transportation and a valid driver’s license.
Essential Functions: Reassessing wounds, providing treatments, supervising staff, documenting patient interactions, coordinating care, and maintaining confidentiality. Strong assessment, communication, and adaptability skills are essential.
