Job Description
Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living settings.
Our team delivers care across the spectrum, from short-term rehab to complex geriatric cases in skilled nursing facilities, aiming to improve access, remove barriers, and achieve optimal clinical outcomes.
As a member of our team, you’ll participate in face-to-face primary care, monthly educational sessions, and collaborate with Nurse Practitioners and Physician Assistants.
