Job Description

Seeking a Full-Time Medical Social Worker for a hospice team in Muskogee.

Provide social and emotional support to patients and their families, collaborating with the healthcare team to develop comprehensive care plans.

Assist with understanding social factors affecting health and coping mechanisms.

Educate staff on community resources and coordinate with hospitals and agencies for continuity of care.

Supervise and mentor other social workers, contribute to policy development.

Qualifications: Bachelor’s in Social Work or Psychology (Master’s preferred), at least one year in healthcare social work, CPR certification, valid driver’s license, dependable transportation.
