Job Description

Seeking a Speech Language Pathologist for our Home Care team to provide personalized assessments and treatments in patients’ homes, enhancing communication, cognition, and swallowing skills.


  • Conduct evaluations using standardized tools and develop individualized, evidence-based care plans.
  • Adjust therapies based on patient progress, caregiver input, and home environment.
  • Document all interventions to ensure compliance and support reimbursement.
  • Educate patients and caregivers on communication strategies and safe swallowing techniques.
  • Collaborate with interdisciplinary teams and participate in care planning meetings.
  • Maintain licensure and stay current with clinical practices.
  • Assist in mentoring peers as needed.

    • Qualifications include a Master’s degree in Speech-Language Pathology, CCC or CFY, NY State license, and CPR certification. Preferred experience includes working with dysphagia and neurogenic disorders.
