Seeking a Speech Language Pathologist to provide personalized evaluation and treatment in patients’ homes, improving communication, cognition, and swallowing for safety and independence.
Responsibilities include interpreting medical prescriptions, conducting assessments, developing and adjusting evidence-based treatment plans, documenting progress, and educating patients and caregivers.
Collaborate with interdisciplinary teams, participate in care planning, and support ongoing professional development.
Requirements: Master’s degree in Speech-Language Pathology, CCC or CFY, NY State licensure, current CPR certification; experience with dysphagia and neurogenic disorders preferred.
