Job Description

Seeking a Speech Language Pathologist for our Home Care team, providing in-home evaluations and treatments to enhance communication, cognition, and swallowing for safety and independence.

Responsibilities include interpreting medical prescriptions, conducting assessments, developing personalized, evidence-based treatment plans, and adjusting strategies based on patient progress.

Providing education to patients and caregivers, documenting care accurately, and collaborating with interdisciplinary teams are essential.

Qualifications: Master’s degree in Speech-Language Pathology, CCC from ASHA or CFY candidate, NY State licensure, and CPR certification. Preferred experience in dysphagia and neurogenic disorders.
