Job Description

Seeking a Speech Language Pathologist for our Home Care team to provide personalized evaluations and treatments in patients’ homes, enhancing communication, cognition, and swallowing abilities.

Responsibilities include interpreting medical prescriptions, conducting assessments, developing and adjusting evidence-based treatment plans, documenting progress, and educating patients and caregivers.

Collaborate with interdisciplinary teams, participate in care planning, and stay current with clinical practices.

Qualifications: Master’s in Speech-Language Pathology, CCC or CFY, NY State license, CPR certification; experience with dysphagia and neurogenic disorders preferred.
