Seeking a Speech Language Pathologist for our Home Care team to provide individualized evaluations and treatments in patients’ homes, focusing on communication, cognition, and swallowing to promote safety and independence.

Responsibilities include conducting assessments, developing evidence-based care plans, adjusting strategies based on progress, and documenting all interventions in compliance with regulations.

The role emphasizes strong patient and caregiver education, interdisciplinary collaboration, and maintaining licensure through continuing education.

Requirements: Master’s degree, CCC or CFY, NY State license, CPR certification; preferred experience with dysphagia and neurogenic disorders.
