Job Description

Seeking a compassionate Nurse Practitioner to provide person-centered primary care for older adults as part of an interdisciplinary team.

Responsibilities include conducting exams, diagnosing and treating illnesses, coordinating 24-hour care, participating in care planning, and managing medical orders.

Requirements:

  • Master’s in Nursing and NP licensure in Michigan
  • Minimum 1 year experience with elderly or frail adults
  • Strong communication skills and proficiency in EMRs and MS Office
  • Ability to work independently and in teams
  • Willingness to work in various environments, including homes and outdoors
  • Full vaccination and willingness to participate in on-call rotations
