Job Description

Vantage Healthcare is seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living facilities.

We serve a diverse patient population, from short-term rehabilitation to complex geriatric cases, aiming to improve access, remove barriers, and deliver state-of-the-art care for optimal outcomes.

As part of our team, physicians will deliver face-to-face primary care, participate in educational sessions and outcomes reviews, and collaborate with nurse practitioners and physician assistants.
