Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA working independently to deliver comprehensive medical care in PALTC settings.
Their goals include improving patient outcomes, enhancing the healthcare experience, reducing preventable hospitalizations, and lowering costs.
Key responsibilities involve conducting exams, diagnosing and treating illnesses, developing care plans, and providing preventive services. They work independently within scope, collaborating with facility leadership and interdisciplinary teams, and maintaining compliance with regulations.
They oversee infection control, patient safety, and health education, demonstrating strong clinical decision-making and communication skills. Proficiency with EHR and telemedicine is required. The role demands physical stamina and prior experience, with a BS-PAS/MPAS/DMSc/MSN-NP/DNP degree, 2+ years of experience, and board certification.
