Job Description
We are seeking a Home Health Speech Language Pathologist to conduct home visits, assess, and treat speech, language, social communication, cognitive, and swallowing disorders.
Perform initial assessments, develop and revise individualized care plans, and coordinate with healthcare team members.
Monitor patient progress, document visits, and report to physicians, ensuring timely re-evaluations every 30 days.
Educate patients and caregivers to promote independence and understanding of impairments.
Qualifications include a Master’s degree, active SLP license (CCC), and at least one year of experience, with home health documentation skills.
