Palliative Care Nurse Practitioner

Job Description

We are seeking a full-time Palliative Care Nurse Practitioner (NP) in Oak Creek, WI, to join our compassionate hospice team dedicated to providing personalized, family-like care.

Develop and manage a caseload of palliative patients, delivering comprehensive care including diagnosis, treatment, medication management, and assessments.

Travel to patients’ homes and facilities to ensure comfort and dignity at the end of life.

Maintain expertise in hospice and palliative care practices.

Collaborate with a multidisciplinary team to support patients and their families.

Requirements: Valid APRN and RN licenses, NPI number or ability to obtain one, and willingness to collaborate with a medical director. Preferred experience includes 1-2 years in hospice or home health. A driver’s license, vehicle, and controlled substance license are also preferred.
