Wound Care Nurse Practitioner

Job Description

The Wound Care Nurse Practitioner provides advanced wound care to homebound and long-term facility patients across Southern California.

Requirements include:

Graduation from an accredited NP or PA program with active California license

Board certification with evidence provided

Wound care experience or certification; preferred experience in hospice, home health, palliative, or assisted living settings

Ability to travel throughout Southern California, lift 50+ lbs, and reposition patients

Strong communication skills, proficiency with medical equipment, and knowledge of regulations

Full mobility and a valid driver’s license

Responsibilities involve performing physical exams, wound assessments, debridement, applying wound care products, documenting treatment, patient education, and collaboration with the healthcare team. The role requires travel, flexible scheduling, and provides compensation per visit ($200-$500).
