Job Description
This full-time position provides short-term therapeutic counseling and crisis intervention for patients and families with psychosocial needs, focusing on developing comprehensive discharge plans for high-risk populations to ensure exceptional care experiences.
- Conduct psychosocial assessments within 72 hours of admission and develop/update Master Treatment Plans accordingly.
- Coordinate care, facilitate discharge planning, and support care transitions, ensuring timely communication with the healthcare team.
- Deliver individual, group, and family therapies, along with crisis intervention and community resource planning.
- Act as a resource for staff, maintain thorough documentation, and stay updated on relevant policies, regulations, and reimbursement sources.
- Collaborate with the interdisciplinary team, support patient and family engagement, and promote positive customer relations.
- Ensure confidentiality, adhere to legal and ethical standards, and participate in ongoing education.
- Assist with risk mitigation, quality improvement initiatives, and fulfill other assigned duties to enhance patient outcomes.
