This full-time physician position offers a $25,000 sign-on bonus and a competitive salary of $300,000–$325,000 annually.
Join the Priority Care Department to support patients with complex medical needs through intensive, coordinated care aimed at improving health outcomes and quality of life.
Manage patients transitioned from primary care or hospitalization, focusing on stabilization, education, and smooth return to their primary provider.
Collaborate with nurse case managers, medical assistants, and a multidisciplinary team to deliver patient-centered, proactive care, aiming to reduce hospitalizations and emergency visits.
Responsibilities include frequent provider visits, comprehensive case management, and resource coordination, including pharmacy and hospice support.
