Seeking a dedicated Primary Geriatric Therapist to provide individual, group, and family therapy to adults in a nursing home setting within PHP/IOP programs.
Develop, implement, and document personalized treatment plans in collaboration with an interdisciplinary team.Provide therapeutic activities to enhance emotional well-being and quality of life for geriatric clients.Ensure compliance with state regulations and maintain accurate documentation in the EHR system.Participate in clinical reviews and care coordination efforts.Qualifications include a Master’s in Social Work, Counseling, or related field, with active Texas licensure (LCSW, LPC, LMFT, etc.).
