*Position Summary*
We are seeking a compassionate and skilled Licensed Nurse Practitioner (NP) to join our growing Palliative and Hospice Care Team. The NP will provide palliative consults and follow-up visits as well as hospice face-to-face evaluations and clinical visits in accordance with state, federal, and company guidelines.

*Key Responsibilities*
* Conduct palliative care visits to assess symptoms, manage pain, and develop individualized care plans that align with patient goals and values.
* Complete hospice visits, including face-to-face evaluations and recertifications per Medicare and organizational guidelines.
* Collaborate with physicians, nurses, social workers, and the interdisciplinary team to ensure holistic care for each patient.
* Provide education to patients, families, and caregivers regarding disease progression, treatment options, and comfort measures.
* Document assessments, interventions, and outcomes accurately and in a timely manner.
* Maintain compliance with all applicable regulatory and clinical standards.
* Participate in team meetings, quality improvement initiatives, and staff education sessions.
* Promote the mission, values, and philosophy of First Health Palliative and Hospice at all times.
*Qualifications*
* Current and active *Nurse Practitioner license* in the State of [Insert State(s)]
* *Board Certified* in Family, Adult, Acute Care, or Geriatric Medicine preferred
* *Minimum 1 year of experience* in palliative care, hospice, internal medicine, or related field
* Strong communication, clinical, and interpersonal skills
* Compassionate approach to patient and family care
* Valid driver’s license and reliable transportation

Pay: $94,000.00 – $110,000.00 per year
